Street to Home
Street to Home assists people who are sleeping rough to access stable accommodation and address the issues surrounding their homelessness.
UnitingCare West’s Assertive Outreach Team works to engage with individuals who are not accessing traditional homeless services, and aims to build trusting relationships in which additional support can be provided to address a range of life issues. The Assertive Outreach team works to engage with individuals within the Tranby Day centre and also on the streets of Perth.
