Street to Home
What is Street to Home?
Assisting people who are sleeping rough to access stable accommodation and address the issues surrounding their homelessness. The Street to Home Team works with individuals who are not accessing traditional homeless services, and aims to build trusting relationships in which additional support can be provided to address a range of life issues.
Street to Home works with people who use Tranby Centre, UCW’s homelessness support hub, and also on the streets of Perth.
Street to Home is an interagency, integrated response to the accommodation, general health, and other support needs of people sleeping rough. People sleeping rough are assisted to access stable accommodation, and address issues surrounding their homelessness to achieve positive, long term outcomes.
Street to Home works alongside the Mental Clinical Outreach Team and liaises with Supportive Housing workers to access and maintain suitable accommodation.

Visit the Tranby Centre at our Inner City Service Hub, or, if you are able, give us a call. You do not need a referral and we will be here to listen to your story.
